The Case Manager completes an initial assessment with the client and develops a Case Plan to determine appropriate activities/goals for the client and his/her family. Directs funding to non-profit organizations dedicated to helping low-income and poor families across New York State. The Community Services Block Grant (CSBG) Program's primary purpose is to assist communities in implementing locally designed anti-poverty programs. The client is assigned a Case Manager for the duration of his/her participation in the FSSP. Participation in the FSSP is based on the family’s income level as well as the availability of Case Manager slots. Under the FSSP, an eligible client is provided services through ongoing case management with the ultimate outcome being the enhancement of a family’s ability to, ultimately, meet their own needs. Such services are delivered through ALPI staff as well as subcontracts with local community organizations.ĪLPI’s focus of CSBG funding is to support a Family Self-Sufficiency Program (FSSP) however, there are limited emergency assistance funds available to help alleviate an immediate crisis. The CSBG Block Grant is the sole source of federal funding for Community Action Agencies that provide a wide range of services and programs to assist needy. As such, ALPI receives Community Services Block Grant (CSBG) funds to assist in the provision of a range of services and activities that have a measurable, and potentially major, impact on the root causes of poverty in the local community. Community Services Block Grant (CSBG) is administered at the federal level through the US Department of Health and Services (HHS), Office of Community. The Community Service Block Grant Self-Sufficiency Case Management Program assists individuals and families in setting goals, learning skills and accessing services needed to remove barriers and to help customers become self-sufficient. CSBG income eligibility levels are currently set at 200 percent of the Federal Poverty Guidelines, as defined. Please contact your local community services agency to determine your eligibility. Low Income Household Water Assistance Program (LIHWAP) The CDBG program improves the economic, social and physical environment of eligible, rural cities and counties to enhance the quality of life for low- and moderate-income residents, thereby strengthening the entire community.
